LEGAL STATEMENT & PRIVACY POLICY

Management Health Solutions, Inc. has created these Legal Statements in order to demonstrate our firm commitment to privacy. The following discloses our information gathering and dissemination practices for all sites under mhsinc.com.

Information Collection and Use

We require customers who register to use the services offered on our Site (collectively, the “Service”) to give us contact information, such as their name, company name, address, phone number, and email address, and financial qualification and billing information, such as billing name and address, and the number of users within the organization that will be using the Service. At the time you express interest in receiving additional information, or when you register for the Service, we may also ask for additional personal information, such as title, department name, fax number, or additional company information, such as annual revenues, number of employees, or industry. Customers can opt out of providing this additional information by not entering it when asked.

Management Health Solutions uses the information that we collect to set up the Service for individuals and their organizations. We may also use the information to contact customers to further discuss customer interest in our company, the Service that we provide, and to send information regarding our company or partners, such as promotions and events. Customers are invited to receive an email newsletter by providing an email address. Customer email addresses and any personal customer information will not be distributed or shared with third parties. Customers can opt-out of having their information used or being contacted by Management Health Solutions, at any time. Separately, customers are also asked to provide an email address when registering for the Service, in order to receive a username and password. We may also email information regarding updates to the Service or company, and will send a Customer Newsletter. Again, email addresses will not be distributed or shared and customers can opt out of receiving any communication at the time it is distributed, or at the time any customer registers for the service. Customers who opt out will still receive administrative emails from Management Health Solutions.

Except as we explicitly state at the time we request information, or as provided for in the licensing agreement between you and Management Health Solutions (the “Licensing Agreement”), we do not disclose to third parties the information provided unless required to do so by laws, regulations or other legal requests for such information or as is necessary to identify, contact or bring legal action against you in the event that we believe you are in violation of the Licensing Agreement. All financial and billing information that we collect through the Site is used solely to check the qualifications of prospective customers and to bill for the Service. This billing information is not used by Management Health Solutions for marketing or promotional purposes. Customers of the Service will be using the Site to host data and information (“Data”). Management Health Solutions will not review, share, distribute, print, or reference any such Data except as provided in the Licensing Agreement, or as may be required by law. Individual records may at times be viewed or accessed only for the purpose of resolving a problem, support issue, or suspected violation of the Licensing Agreement, or as may be required by law. Of course, customers are responsible for maintaining the confidentiality and security of their user registration and password.

Management Health Solutions may also collect certain information from visitors to and customers of the Site, such as Internet addresses. This information is logged to help diagnose technical problems, and to administer our Site in order to constantly improve the quality of the Service. We may also track and analyze non-identifying and aggregate usage and volume statistical information from our visitors and customers and provide such information to third parties.

If a user elects to use our referral service for informing a friend about our site, we ask them for the friend’s name and email address. Management Health Solutions will automatically send the friend a one-time email inviting them to visit the site. Management Health Solutions does not store this information.

Information Request Form

If a user wishes to receive any Management Health Solutions collateral or other materials, they may be required to provide contact information including name, address, and telephone number(s). Unless otherwise indicated, this information can be used to contact the user.

No Warranty

This site, its contents and the data and information which may be accessed through this site are provided “as is”; Management Health Solutions makes no representation or warranty of any kind, whether expressed or implied, regarding this site, any site accessible through this site, or the accuracy or completeness of the data and information which may be accessed through this site. Management Health Solutions expressly disclaims all express or implied warranties including, but not limited to, any warranty of fitness for a particular purpose, title, fiscal benefits, and non-infringement. In no event will Management Health Solutions be liable to any party for any money damages (whether direct, indirect, special, incidental, or consequential) incurred due to that party’s use of this site or data supplied to that party through this site.

Privacy Policy

The Site contains links to other websites. Management Health Solutions is not responsible for the privacy practices or the content of these others web sites. Customers and visitors will need to check the policy statement of these others websites to understand their policies. Customers and visitors who access a linked site may be disclosing their private information. It is the responsibility of the user to keep such information private and confidential.

Log Files

Management Health Solutions may use IP addresses to analyze trends, administer the web site, track user’s movement, and gather broad demographic information for aggregate use. IP addresses are not linked to personally identifiable information.
